Worst Case Scenario: When Viagra is prescribed by a doctor for a genuine medical problem, the dose will be clearly specified. Taken recreationally, however, the only counsel you receive may be from a buddy recounting the tale of his indomitable boner during an epic sex session. That might lead some to jump to the conclusion that more Viagra means more good times—people like Daniel Medforth—a Brit who in 2015 choked down 35 pills at the end of a boozy two-day bender "for a laugh."

Overall health. Certain existing conditions can affect how long Viagra lasts and how well it works for you. Diabetes, nervous system conditions like multiple sclerosis (MS), and heart conditions like atherosclerosis (fat buildup in your blood vessels) can all make Viagra less effective and not last as long. Some kidney conditions may make Viagra last longer because of the condition’s effect on your metabolism.
It can be frustrating trying to determine why you aren’t experiencing the desired results but there is usually a reason, so consider the preceding factors and arm yourself with this knowledge as you speak with your GP. There are different things you can do to help bring about the results you’re hoping for, just remember that success with ED isn’t always instantaneous.

There was a higher incidence of adverse events in those taking niacin. However, most patients could tolerate it at the maximum dosage (1,500 mg/day). With this in mind, niacin could be an alternative choice of treatment for patients with ED. Despite the success of phosphodiesterase type 5 inhibitors (PDE5 inhibitors), such as sildenafil, only around 60–70% of patients have a satisfactory response to this class of drugs. And there are adverse effects such as headache, flushing, dyspepsia, nasal congestion, and impaired vision, including photophobia and blurred vision. Hence, there is a need to develop other therapeutic agents for those patients who do not respond satisfactorily to PDE5 inhibitors or are contraindicated for those such as sildenafil.
Long considered an aphrodisiac by the Chinese, ginseng may do more than just rev your engine. A 2013 South Korean study found that taking the herb for just a few weeks improved guys’ performance in the bedroom, including helping them last longer before finishing. Meanwhile, a study in Spermatogenesis found that ginseng can also help make for harder, longer-lasting erections and improve testosterone levels, which in turn boosts libido. “Ginseng is a promising herbal therapy for ED because it helps promote relaxation of smooth muscle in the penis, increase dopamine levels in the brain, and increase pressure in the cavernosal nerves of the penis which helps nitric oxide synthesis,” Walker explains.
In fact, dyslipidemia is commonly found in ED patients, and studies show that statins can help to improve the response of PDE5 inhibitors in those suffering from ED precisely because they improve atherosclerosis. Consequently, statins can be used as a treatment in patients with an unsatisfactory response to PDE5 inhibitors, yet there are problems with statins too, among them raised liver enzymes and muscle problems, some of which can be quite serious and even deadly (rhabdomylosis).
Intracavernosal injections remain an excellent treatment option with proven efficacy and safety over time. Baniel and colleagues (2001) treated sildenafil non-responders after radical prostatectomy with intracavernosal injections of vasoactive drugs with 85% success rate. Shabsigh and colleagues (2000) were able to salvage sildenafil non responders with intracavernosal injections of alprostadil. More than 85% improvement in Q3 and Q4 of the IIEF was recorded (Shabsigh et al 2000). The tri-mix combination (papaverine, phentolamine, alprostadil) is certainly the more efficacious drug treatment for erectile dysfunction and can be used in severe cases of erectile dysfunction when intravernous alprostadil fails or it is not preferable due to side effects.
The group treated with the lower concentration of zinc (1 mg/day) did not show an alteration in any of the observed parameters. However, supplementation with a dose of 5 mg/day per rat caused substantial prolonged ejaculatory latency and increased in number of penile thrusting. The other parameters studied remained unchanged indicating uninterrupted libido, sex vigor and performance. Majority of male rats (75 %) showed the prominent actions of sexual behaviour (mount, intromission and penile thrusting) and did not ejaculate within the 15-minute observation period.
Minor side effects associated with erectile dysfunction medications include indigestion, runny nose and skin flushes. Sildenafil may cause temporary changes in your vision, and vardenafil may cause muscle aches and back pain. More serious side effects include hearing loss and erections that last longer than four hours. If you have cardiovascular problems, including a history of heart attacks or strokes, high or low blood pressure or vision problems such as retinitis pigmentosa, it may prove unsafe for you to take sildenafil, vardenafil or tadalafil. These medications may also interact poorly with some antibiotics, blood thinners and medications to prevent seizures and heart rhythm disorders.
Pelvic floor PT is a proven and effective treatment for erectile dysfunction. A study in 2006 involved 146 men with mean age of 42 with pelvic pain; 92% of these men had sexual dysfunction.  Treatment consisted of trigger point release and relaxation training, which led to improvement in symptoms by 77-87% – it’s notable that kegels were not part of this study.
A drug called Alprostadil, either alone or sometimes in conjunction with others such as papaverine and/or phentolamine, can be injected directly into the penis to dilate the arteries, experts say. The drug produces an erection in about 10 minutes that can last up to an hour. But there are several disadvantages, says Teresa Beam, MD, a urologist with the Grey Clinic. Some patients are averse to using a needle, which is why many men abandon the therapy. Those who give it a try may experience pain at the injection site or priapism (a painful erection lasting too long).

Keep yourself healthy by consuming these vitamins and a healthy diet that is high in fibre and nutrients. Try to ignore OTC medicines like I-Arginine and other pills, and start including these fresh vegetables, apples, oats and whole grains in your daily life. 
Two years ago I took regular Niacine for about a year to lower LDL and increase HDL. I did not want to take Statins because of its side effects. I was being monitored by my Dr. because of the effect on liver enzymes. I took 1.5 gr together with Phytosterols. The treatment was effective and the only side effect were the flushes which I found could be eliminated by having 500 mg at the end of each of the 3 main meals. I stopped treatment for a year or so, but now the Dr. suggested I start taking Niacine. or Statins. I chose Niacine (Nicotine Acid) and started with 500 mgs for 3 days; increased it to 1000 mgs. for 4 more days, until I increased it to 500 mgs x 3 for a total of 1.5 grms/day taking 500 mgms/meal. I started noticing my gradual decrease in libido this time almost inmediately. I do not take any other medicines as such I'm definitely inclined to blame Niacine because I have taken Phytosterols for 3 years and my libido was fine. I'm a senior. Hope this will help!

The power of the mind is not to be underestimated when it comes to believing in remedies so they may sometimes work, even if they do not contain any active substance (this is known as a placebo effect). However, herbal ED remedies offered online can present major health risks. It is important that you understand your treatment options before you take any medication.
Similar results were presented by other studies. Incorrect administration accounted for 81% of sildenafil failures referred by primary care physicians (Atiemo et al 2003). Reasons included administration after heavy meals, lack of sexual stimulation, short timing to intercourse, and too few attempts for intercourse. The authors were able to salvage 41.5% of nonresponders after reeducation as measured by the General Assessment Question (GAG) and Sexual Health Inventory for Men (SHIM) scores. Gruenwald and colleagues (2006) showed that counseling and dose adjustment were directly influential in achieving an excellent response to a second trial of sildenafil in patients with ED who had previously failed treatment with the drug, and obviated their needing to seek more invasive measures. In this study, 38% of the included patients did not receive any explanations or counseling whatsoever from the prescribing physician. After proper instruction, erectile function domain scores increased significantly and overall 23.6% of the patients had a normal IIEF EF domain score (≥26) at the end of the study.
In addition to getting helpful nutrients from your diet, you can also consider supplementing. It’s a good idea to check in with a practitioner, particularly one with knowledge of your individual health issues and medications, before taking anything. Here are some I recommend:
For Men: Soy is terrible for the male sex drive as the phytoestrogens build up to unnatural levels much too quickly. In fact, an ex boyfriend and I experimented with this when he decided to become vegan. He ate and drank soy products for a month and by the end of that time he had zero libido. Within two weeks of stopping all soy, he was back to normal.
For many men, stopping smoking is an erectile dysfunction remedy, particularly when ED is the result of vascular disease, which occurs when blood supply to the penis becomes restricted because of blockage or narrowing of the arteries. Smoking and even smokeless tobacco can also cause the narrowing of important blood vessels and have the same negative impact.
